My wife crashed on rollerblades and had the same problem you have. She
went thru physical therapy, pain shots, nerve conductivity tests (twice),
and a bunch of other stuff. It came down to something called
"piriformus syndrome", where, for whatever reason, the piriformus muscle
is irritating the sciatic nerve and causing problems. Some people are
born with the nerve going thru the muscle instead of under it. She
was ok as long as she didn't do anything athletic, like volleyball,
cycling (on a tandem), aerobics, etc. If she did something like that,
she'd be in pain for hours or days, all the way from thigh to ankle.
Most of the doctors gave up at this point, since they had no idea what
piriformus syndrome was, and if they did, they didn't want to mess around
with large nerves. Her ortho finally found a doctor in Los Angeles that
would do the surgery to move the piriformus, and although she's not 100%,
she can do a lot more than she could before.
