dont do it, my neurosurgon says its a scam, just a overpriced "drug cocktail" thta can have limited results at best, hell they want your credit card number just to make a "free appointment " consultation ,and then threaten to charge $100 from your credit card if you dont show for your appointment. Ive heard that single injection is very costly.. I would look at finding the cause of your pain rather than over priced medication to mask it....
